If you are enrolled in medical, dental, and/or vision benefits, your coverage will remain active through the end of the month in which your contract ends. You will then have the opportunity to continue these benefits through COBRA at your cost. COBRA information is sent from BASIC, our COBRA Administrator via U.S. Mail. 

• All other policies, including group term life insurance, short-term and long-term disability, voluntary life insurance, accident coverage, critical illness coverage, and hospital indemnity coverage, terminate the day your contract ends.
• If you are enrolled in a 401k, contact the Benefits Administrator using the link below to request a 401k rollover form.
